I was playing Metroid Fusion for the GBA, and in one room , The SA-X(An evil clone of Samus) came in as i was walking out. 5 seconds later, I died.

Oh, I remeber that! She could kill you in three blast, easy!
Everytime the SA-X would show up, my heart would start racing, and I could not push the control pad/weapon-firing button fast enough.
And when you finally get away from her, and you watch her walk off, you just kinda' sit there thinking, "Holy , I never want to do that again."
.....but, because she was such an evil, hard-ass, beating her at the end of the game was all the more thrilling.

I GOT SCARED OF HIM TOO! My heart started racing and he made it look like he resetted the game and I got scared there too cause I thought he deleted my file. Thank God he didn't.

When I played Super Mario Bros. 3 (Super Mario Advanced 4) on the GBA when I was younger, the part when you get the letter from Bowser when you entered World 8 used to scare the crap outta me.

The wall master in ocarina of time used to really spook me for some reason, and one of those old casper games used to get me when I was a kid.
I don't own it, but try playing F.E.A.R. Its a Survival horror game. It was just a little bit of gameplay on youtube, but it scared a year off my life. I swear.

Zelda:OoT Dead Hands. And under the karkariko well. Like Likes too.
DEAD HANDS ARE SO CREEPY.
Like you'd go in, get stuck by a hand, then this weird thing would shuffle/waddle toward you, then rear up and lunge real fast and start sucking at young link's head...
And then the really dark darkness of it didn't help at all.
Shadow temple wasn't actually that bad for me, cause dead hands are so much less scary when Link is an adult.
Like likes scared me too. I was always so afraid that link would get swallowed by one...
Also Morpheus in the water temple was kinda freaky. Gohma used to frighten me when I first played it, but I got over that quickly when I figured out how to stop her from dropping onto link.
OOOOH The Skulltula house in Karkariko Is FREAKY. You'd go in and this half spider half person drops down suddenly in front of you.... eep!
Wall Masters are never fun. Floor Masters are though.
Oh yea, The Forest Temple was kinda freaky when I first played it...
In Twilight Princess, in the Cave of Ordeals, I always get a little nervous when jumping into a dark room...
And what is slightly freaky is the invisible rats that cling to wolf link... if you use sense and look at them, man they is UGLY.
Oh yea, Majora's mask was just... brrrrrrr.... The moon gave me a nightmare.... All smily...
Half-Life 2, that level with all the headcrabs and zombies... brrr.... Once you got used to the blood and stuff it was kinda fun, cept when the friggin poison headcrab jumps outta nowhere and bites your face off.

I'm not usually frightened by games, I'm usually only hit by a shock when something creepy occurs.
Like Marx Soul in Kirby Super Star Ultra. When I finally beat him, he vanished, reappeared at the top of the screen, let off a horrible, blood-curdling shriek, split in half and exploded. It was kinda weird at first, but now whenever I beat him it's entertaining. :)
Overall the Kirby series is really dark. 0² (Zero Two) still makes my skin crawl whenever you blast his eye and blood spews out. And when the Nightmare in Kirby's Adventure (NES version of Nightmare in Dreamland) explodes, it happens with a scratchy NES sound effect that really stuns you if you don't expect it.
And I have to admit that the Shadow temple in Ocarina freaked me out. And the forest temple was eerie as well.

When I was young, and the PS1 still had games selling, there was a game, ugh what was the name? Robo Pit 2! Near the begining you get challeneged by a black robot and stuff with fire punches and it talks about ripping you apart. It looked scary. Did I mention it forced the challenge to happen at night?

Well, I freaked out playing Wind Waker and OoT. Sneaking past the Moblin guards was real nerve wracking, and crawling around the corners in the Forest Temple, waiting for a Floor Master to kick your at any moment... yeep!
I actually kinda freaked out when I found Suicune in Burned Tower, thinking I had to fight it with my Pokemon all at level-red HP... LOL

The Old Chateau in Pokemon Diamond, and all of the Genome Cyborg game. *shudder*
oh my god I forgot about Tomb Raider. I was more afraid of the creepy butler in Tomb Raider 2 when you ran around Laura's house. He followed you everyone and would just magically show up out of nowhere.
Also, The Incredible Hulk for PS1. That game was just eerie because it was always dark probably because the PS1 couldn't handle everything going on in the game.
Tomba 2 for PS1 is another one, especially at the end when the final pig freezes time. Everything goes black and white, it gets very dark, nothing moves, and the music is very creepy.
Another good one is Friday the 13th for the NES is another good one. When you're in the houses and you're not sure if Jason is in there or not, then you turn around a corner and plays that "CHUN CHUN CHUN" sound really loud. Startles me everytime.

I'm kinda jumpy so a lot of stuff scares me. But the scariest things to me in any game are zombie dogs. They scare the **** out of me.

In Metroid fusion, whenever SA-X appears throughout the game I get so freaked out, my hands tremble. Its really nerve raking when I have to fight SA-X at the end of the game <.<

I've beaten the game three times as well (working on four and five, respectively) and every time I hear that "infant-with-a-cold" wheezing, My heart races and my hands get all shaky...
They slowly shamble towards you with they're exposed piranha mouths, their red eyes a-glow, You get out your rifle with the Thermal Scope and as soon as you lock on to a parasite and squeeze the trigger....
THEY EXTEND THEY'RE ARMS TO GRAB YOU, DRAG YOU ACROSS THE ROOM, AND START GNAWING ON YOUR THROAT WITH THOSE DEVIL-FISH TEETH!!!!
*shudders*
They're worse then any boss, in creepiness and in difficulty....

Same for me too.
A word to the wise, NEVER go into the graveyard at night! When I did this, I couldn't find the exit, and kept thinking I was hearing ghosts.
Now, I never get good luck, it just happened to be something managed to fall off the wall near me, this scared the MissingNos out of me! I had to have my sister finish this part for me! But y'all should have seen the look on my face!

That whole section of Amy's Twinkle Park stage in Sonic Adventure used to frighten me with its creepy music.
Also, the Old Chateau in Diamond/Pearl freaked me out when I went exploring and stumbled upon the ghosts.

i would say playing the Fear games, the scenes when your in the crazy place with Alma just swinging, or in the second one where your walking down the hallway and you blink and she just appears closer and closer til shes just all up in your face trying to kill you. freaked me out. also, when i was a kid i wouldnt fight the spider in ocarina of time. i was little and it scared me to death, my dad did that stuff, just like crossing hyrule field. and Ravenholm (or whatever its called) in Halflife 2? nonono. don't like that. and about the Deathclaws. that scares the crap out of me when your running around and one attacks you and i jump. also i remember playing this Quake 4 game and you were in a vent and there was all this running around like dog things in the vents around you. last time i played that game. im not still scared, but it freaked me out pretty bad. lol. OH. and this is silly, but if you've ever played the nancy drew games, my older sister plays em and i help her with them (cause she sucks at those puzzles.) but one had this thing with werewolves and you had nightmares and stuff (pretty ****ed up.) and the very first one.. you look between some books and theres this bright yellow eye staring back at me. as a kid that scared me.

oh and i just now remembered. this... playing ocarina of time and your walking across that big room with all the water, the tree and the big door at the end, you walk to the door and its locked. you decide to turn around "OH S***" that thing freaked me out even now, the evil link. just turning around and BOOM hes there. creepy. he was very hard to kill as well.

The Gygas boss battle in Earthbound was the most insane thing in the entire video game world. First he turns into a fetus and then he starts yelling your characters name. Battle him durring the daytime, because at night, you will pee. Garunteed.

The only games to have scared me were all Doom games, the original Doom on PC scared me the first time I ran into a bunch of Hell Knights, this was when I was pretty young, and now playing through the game I just laugh at them and proceed to continue the gore-fest. Doom 3 also scared me, having been a major update to the old game it proved to be more scary than all of its forefathers, however at this point I wasn't really scared of the creatures themselves, I was scared because they would always jump out of the shadows when you least expected it.
No other series has managed to scare me like Doom, but there have been plenty of moments that made me jump, Fallout 3 has given me quite a few of these with various enemies following you, turn around and WHAM you just got hit in the face by a Super Mutant, Deathclaw or Yao-Guai.
